Below is our onboarding form, which contains the information we need from you to begin the onboarding process. We’ve included some additional information on each of the items below, in case you need some guidance.

Here’s what we need from you to get things started:

Desired URL for your Registration Portal

This is the link that your members will use to access your registration portal and register, view schedules, access coach/manager tools, etc., and also where administrators will log in to access their admin tools.


This is always in the following format: clubname.powerupsports.com

You’ll need to enter the desired URL you wish to use.


For example, Demo City Football Club might use: democityfc.powerupsports.com

Club Logo

We’ll need a high quality version of your club’s logo for use in the registration portal.


An overview of the programs

An overview of the programs you plan to offer for your next registration offering.

This will help our team to guide you through the first implementation and anticipate questions, so we can make the most of your time in the initial training session. Our preference is for this to be submitted in spreadsheet form.


Details we’re looking for:

  • Program Name
  • Age Restrictions
  • Price
  • Spaces available


Copy of your current registration form


We use this to create the necessary waivers for your registration. If you don’t have a registration form, please submit a copy of your current (or desired) waivers. This needs to be in a form that we can copy/paste the text from (e.g. Word document, PDF).

Some examples of what might be included are:


  • Liability Waiver
  • Photo/Video Consent
  • Refund Policy
  • Uniform Size
  • Volunteer Roles/forms/details


Details of any Discount Policy

This is to determine if it is possible to implement the discount rule/policy.

Target “Go Live” Date for the Registration Site

This is the day you’re aiming to launch your new registration portal to your members.

Direction of “Customer Pay” Activation

You’ll need to let us know if you’d like Customer Pay activated (customer pays the processing fees) or not (club absorbs the cost).
